Emerging Leader in APTA Home Health

Rationale

The purpose of this award is to recognize and honor one Physical Therapist or Physical Therapist Assistant that has demonstrated extraordinary service and clinical passion early in his or her home health career. The individual should have made exceptional overall accomplishments and contributions to the APTA and/or the Home Health Academy mission and vision, and to the physical therapy profession to advance quality and evidenced‐based care in the home.

Criteria

To be eligible for this award, the nominee must:

  1. Be a current member of the APTA Home Health Academy
  2. Have been involved in home health clinical practice settings for a minimum of one year and a maximum of five years
  3. Demonstrate a commitment to innovative, evidenced‐based clinical care, and problem solving in the home setting that leads to improved patient outcomes. This could be seen in many types of roles from population management program development, to clinician scheduling ideas for better time management and outstanding conflict resolution skills for patient satisfaction.

Nomination Process

Any current member of the American Physical Therapy Association may nominate a physical therapist or physical therapist assistant who meets the award criteria. Nomination packets should include:

  1. One or more letters of nomination addressed to the APTA Home Health Academy office describing how and why the nominee meets the criteria.
    1. At least one letter of endorsement (support statement) should be included with the nominating letter and address some, if not all, of the following:
      1. How long has this nominee worked in home health clinical practice?
      2. How does this nominee demonstrate extraordinary service and clinical passion?
      3. List the nominee's accomplishments and contributions to the APTA and/or the Home Health Academy.
      4. Give examples of this nominee's commitment to improved patient outcomes.
      5. Anything else you want to add to support why this nominee should be recognized as an emerging leader in the Home Health Academy?

 

Selection Process

The Nominating Committee will review Nominations and the chair will present the Committee's recommendation to the Home Health Academy's Executive Committee for final approval.

Presentation

The award will be presented following the Business Meeting during APTA Combined Sections Meeting, usually held in February of each year. 

The Award will consist of a $50 gift certificate for the APTA Learning Center.
